NFIRS 5.0 Example Reports

Here are some example NFIRS incident reports demonstrating the variety of information NFIRS captures. NFIRS covers all hazards from fire to medical to MVA and more, and much as it traditionally uses paper forms (now called modules) these days it is regularly captured as digital forms:

Example Group  Description 
Structure Fire Fire report for structure fire; building / home or other structure fire. (example #1).
Vehicle Fire Fire report for vehicle fire; aka ‘mobile property’ fire. (example #2).
Wildland Fire Fire report for wildland fire; i.e. trees, grass & outside fire. (example #3).
Campfire Fire report for campfire (example #4).
Medical Medical report for medical incident (example #5).
MVA with Injuries MVA report for motor vehicle accident; with injuries (example #6).
MVA without Injuries MVA report for motor vehicle accident; with no injuries (example #7).
Search & Rescue SAR report for Search & Rescue (example #8).
Cancelled General report for cancelled - en route or on scene. Stood down (example #9).
Unable to Locate UTL General report for unable to locate / no incident found (example #10).
